GP’s, Health Professionals & NDIS clients

If you would like to refer yourself (NDIS), a client or a patient, please email us directly at imperfectlyyounutrition@gmail.com with your referral details (including a copy of the relevant Medicare Care Plan with provider details), with evidence that the client has given consent for the referral.
